General Tips
- Make sure you fully complete our claim form and send the completed claim form together with the appropriate documents to us as soon as possible. It is important that you remember to provide your contact details especially:
- email address
- cell phone / telephone number
- All claim forms must be completed in English
Medical Claim
- Complete and sign claim form. (Important: One claim form for one medical event).
- Request medical note from your doctor to help speed up your claim process
- Post to P. O. Box 2011, Christchurch the following:
- fully completed claim form and
- original documents including invoices and medical notes (if you have one). All the invoices that you send must include GST number.
- If you need surgery or hospitalisation, complete claim form for pre-approval. We recommend that you request a pre-approval at least five working days prior to surgery or hospitalisation, to help you avoid uncertainty about whether your claims will be covered or not.
Property Claim
- To prove ownership of your stolen or lost property, we prefer original receipts/invoices. However, if your receipts/invoices are:
- overseas, you can first provide copies of the receipts/invoices and send in the original receipts/invoices when they are received from overseas.
- lost, send in the remaining accessories. For example: manuals, adaptors, photos of the items, cases etc.
- You must not dispose of any accessories or items that you wish to claim for.
(A) Burglary, Theft or Lost Property
- Notify the police immediately
- If items have been lost, advise the place where items lost, if possible. For example: Transport operators, shopping mall customer service, etc.
- Complete and sign claim form. (Important: Provide as much description as possible on the property you are claiming for. For example: if claiming for a laptop, provide brand name, model and date of purchase.)
- Post to P. O. Box 2011, Christchurch the following:
- fully completed claim form,
- police report and
- original proof of ownership – documents or accessories
(B) Electronic Goods, e.g. laptops, camera etc
- If claiming for damaged laptop, complete and send in your claim form. We will organize for a repairer to assess the condition and cost of repairing or replacing your laptop.
- If claiming for lost electronic goods, please send any remaining accessories with your claim form. For example: charger, manuals, etc.
- If claiming for damaged electronic goods (not including laptops/computers), please obtain a written report on the damage (must include cause and resulting damage) and quote to repair and submit with your claim form. If the repairer advises the item is not repairable, please send the damaged item, including all accessories, and report with your claim form.
Liability Claim
- Never admit liability.
- Advise the third party to submit a claim to their insurance company.
- If the third party refuses to submit the claim to their insurance company or if they are not insured, provide your claim details to them to contact us.
